Beyond pure roleplay, does this have any impact on me in the long run and for downtime?

As I understand it (and as someone on -4 in the Infirmary at the moment), it has zero impact on your downtime activities. You still your three month slots to play with.
From -2 you'd probably be up and about in a couple of days to a week anyway I think.

Nope Dan, no affect on your downtime.
-2 to Full with a doctor's care and medical treatment back at base is only a few days anyway.
